This is a blanket shaped like a stuffed animal, called Blankie Bear, from the British brand Gagagoogoo.
The blanket is connected to the back of the bear and can be stored there.
When unfolded, it looks like the baby in the first image is holding it. Please also refer to the instruction manual in the fourth image.

It was a popular product, but the company no longer exists, making it very rare.

I bought it as a gift because it's very cute, but I missed the opportunity to give it, and it has been stored in its box (in an environment away from high temperatures and humidity).
